An updated vaccine can add protection against severe COVID, particularly for older adults and people at higher risk.

**The number:** 45% estimated protection against COVID hospitalization in adults 65+, 2024 to 2025 VISION data ([CDC MMWR, 2025](https://www.cdc.gov/mmwr/volumes/74/wr/mm7406a1.htm))

## The evidence

- Evidence grade: B. Grade B: consistent large cohort studies, with some trial evidence.
- Basis: CDC observational effectiveness study and September 23, 2026 clinical vaccination guidance
- Outcome: COVID hospitalization, immunocompetent adults 65+, VISION 2024 to 2025
- Comparison: Seasonal dose 7 to 119 days earlier versus no dose that season
- Impact: 70/100. The impact score is an editorial prioritization heuristic for the general list, not a validated estimate of your personal risk.

## Why it matters

A CDC study estimated 45% protection against COVID hospitalization in immunocompetent adults aged 65 and older in the VISION network during the first 7 to 119 days after a 2024 to 2025 dose, compared with no dose that season. That historical estimate does not predict the exact protection of a newer vaccine. CDC's September 23, 2026 clinical guidance recommends a 2026 to 2027 vaccine for all adults aged 18 and older, with product and dose decisions made with a healthcare professional.

## How to do it

1. Check the current national offer; CDC's September 2026 guidance recommends the 2026 to 2027 vaccine for all US adults.
2. Review your age, health conditions, prior doses and recent infection with a vaccinator.
3. Discuss the expected benefit and timing, especially if you are 65 or older or pregnant.
4. Record the vaccination and the next dose date; CDC recommends 2 doses in this season for adults aged 65 and older.

## Talk to a clinician first

A vaccinator should check immune status, prior myocarditis and product eligibility. Some uses in current CDC guidance are outside FDA labeling, so individual advice matters.

## Putting it into practice

### Start now

- 1 minute: Recall your last COVID vaccine and any recent infection, even approximately, so you can ask about timing.
- 5 minutes: Check your national vaccination service for the current offer and ask a pharmacy or clinic if you are eligible.
- 15 minutes: Gather previous dose dates, relevant conditions and any past vaccine reaction; prepare your questions about benefit and timing.

### What it looks like

- Ask a vaccinator to review your last dose and recent infection before choosing a date.
- If treatment weakens your immune system, ask your specialist how vaccination fits around it.

### Common mistakes

- Last season's eligibility may have changed, so check the current offer in your country.
- A dose years ago does not settle your current needs, so review your record with a vaccinator.

### A structured start

**Review this season's protection**

1. Check the current local offer and your vaccination record.
2. Discuss eligibility, recent infection and any previous serious reaction with the vaccinator.
3. Book if recommended, then record the vaccination and any further date they advise.

## In Czechia

In Czechia, for the 2026/2027 season the Ministry of Health distributes Comirnaty XFG 30 micrograms for people 12 and over from 1 September 2026. The next dose comes at least 3 months after the previous one, with timing set by a doctor.

## In Germany

In Germany, STIKO has recommended a yearly COVID-19 booster since July 2026 for everyone from 75 and for people from 6 months old with an underlying condition that raises their risk. It is best given in late summer or early autumn.

## In Spain

In Spain, COVID-19 vaccination for the 2026 to 2027 season is recommended for people aged 70 or over, care home residents, people from 6 months with serious vulnerability, people from 12 with listed risk conditions, and all pregnant women.

## In Netherlands

In the Netherlands, this autumn's COVID-19 jab is for people 70 and over, people 50 to 70 with a medical condition, and adults and children with a severe chronic condition or immune disorder. Under-70s in these groups get no personal invitation and book via planjeprik.nl or 0800-7070.

## In Poland

In Poland, COVID-19 vaccination is free for people insured by the NFZ, with no e-referral, at a POZ or pharmacy. People 12 and over without reduced immunity get one dose of Spikevax XFG; it is especially recommended over 60 and for other listed groups.

## In Ukraine

In Ukraine, most people not yet vaccinated against COVID-19 need one dose. Extra doses every 6 to 12 months are for people at risk of severe illness: weakened immunity or chronic disease, pregnancy, age over 60, and at-risk jobs such as teachers and doctors. Vaccination is free.
