Clearer communication and the right support can ease strain at home and protect your wellbeing.

**The number:** Behavioural couples therapy runs 15 to 20 sessions over 5 to 6 months ([NICE, 2022](https://www.nice.org.uk/guidance/ng222/chapter/Recommendations))

## The evidence

- Evidence grade: B. Grade B: consistent large cohort studies, with some trial evidence.
- Basis: NICE NG222 recommendation on behavioural couples therapy in depression, plus NHS relationship guidance
- Impact: 49/100. The impact score is an editorial prioritization heuristic for the general list, not a validated estimate of your personal risk.

## Why it matters

Recurring conflict adds to distress and makes it harder to support each other. NHS guidance encourages open communication, listening and boundaries. When relationship problems contribute to depression, NICE recommends considering behavioural couples therapy, delivered over 15 to 20 sessions across 5 to 6 months. None of this means staying in a relationship that is harming you.

## How to do it

1. Pick a calm time to say how you feel, then listen to the other person's version.
2. Agree boundaries, and pause a discussion when it gets too heated to be useful.
3. If the same problems keep returning, ask a clinician or service about appropriate counselling or therapy.

## Talk to a clinician first

If there is fear, coercion or abuse, seek confidential specialist support. Joint counselling can be unsafe in that situation.

## Putting it into practice

### Start now

- 1 minute: Privately name one feeling and one need: "I feel overwhelmed and need help with dinner." Use it only in a safe, calm conversation.
- 5 minutes: If the relationship feels safe, prepare one issue to discuss and one clear request; choose a calm time together.
- 15 minutes: If safe and both willing, discuss one issue: take turns speaking, reflect back what you heard and agree one small change; pause if heated.

### What it looks like

- Say "I feel overwhelmed by the evening chores; can we agree who handles dinner?" at a calm time, if it feels safe.
- Check your understanding: "It sounds like you felt left out. Have I understood you?"

### Common mistakes

- Trying to settle a disagreement while angry can escalate it: pause and return when you both feel calmer.
- Fear, coercion or abuse needs confidential specialist help: do not use these exercises or joint counselling to confront an unsafe partner.

### A structured start

**Start a safer, clearer conversation**

1. First consider safety: if you feel afraid or controlled, seek confidential specialist support instead of a joint discussion.
2. In a safe relationship, choose a calm time and exchange one concern and one practical request.
3. Check whether the agreed change helps; if conflict keeps returning, ask about suitable counselling.
