A cholesterol result is useful when it leads to a plan based on your overall risk of heart attack and stroke.

**The number:** About 28% fewer cardiovascular events with statins in adults at increased risk ([USPSTF, 2022](https://www.uspreventiveservicestaskforce.org/uspstf/document/final-evidence-summary/statin-use-in-adults-preventive-medication))

## The evidence

- Evidence grade: A. Grade A: meta-analyses of randomised trials, or a major guideline consensus.
- Basis: USPSTF 2022 review of 22 statin trials versus placebo or no statin, plus 2026 ACC/AHA guidance
- Outcome: cardiovascular events in adults at increased risk
- Comparison: Statin treatment versus placebo or no statin; not the effect of testing
- Impact: 83/100. The impact score is an editorial prioritization heuristic for the general list, not a validated estimate of your personal risk.

## Why it matters

Cholesterol helps identify people who could benefit from prevention before a first cardiovascular event. A USPSTF review found that statins reduced combined cardiovascular events by about 28% in adults with increased risk, with an absolute reduction of 1.28 percentage points over 1 to 6 years. That is a treatment effect, so testing needs to be followed by an individual risk discussion.

## How to do it

1. Ask whether a cholesterol blood test is due and agree on repeat testing based on your risk.
2. Bring your blood pressure results and family history of early heart disease to the review.
3. Review overall cardiovascular risk using a local clinical tool, and ask about the once in adulthood Lp(a) test in the 2026 ACC/AHA guidance.
4. If treatment is recommended, discuss likely absolute benefit, side effects and follow-up.

## Talk to a clinician first

A cholesterol test does not automatically mean you need medicine. Pregnancy, very high cholesterol and established heart disease need individualized care.

## Putting it into practice

### Start now

- 1 minute: Recall any close relative with early heart disease and their approximate age; this is useful at your cholesterol review.
- 5 minutes: Ask your clinic whether a cholesterol test is due and how to arrange it.
- 15 minutes: Gather your last cholesterol and blood pressure results; list medicines and any family history of early heart disease for the review.

### What it looks like

- Tell your clinician if a parent or sibling had heart disease unusually young.
- At a cholesterol review, ask what your result means alongside blood pressure and smoking history.

### Common mistakes

- A cholesterol number alone cannot decide treatment, so ask about your overall heart risk.
- Most people do not need to fast for a cholesterol test, so follow the instructions you are given rather than skipping meals.

### A structured start

**Make the result useful**

1. Ask whether testing is appropriate and get preparation instructions.
2. Have the agreed test and find out how you will receive the result.
3. Review overall risk and ask about the absolute benefit and possible harms of any proposed treatment.
4. Agree when to review the plan and repeat testing.

## In Czechia

In Czechia, the GP preventive check tests blood lipids at 25, every 4 years from 30 and every 2 years from 40. Lp(a) is tested once in life, and once more for women after menopause.

## In Germany

In Germany, statutory insurance covers a Gesundheits-Check-up once between 18 and 34 and every 3 years from 35. It includes a blood lipid test, which at 18 to 34 is only done if you have risk factors.

## In Poland

In Poland, the free Moje Zdrowie programme includes a lipid profile, plus a one-time Lp(a) test at ages 20 to 40. It is available every 5 years at 20 to 49 and every 3 years from 50; start with the questionnaire in IKP, mojeIKP or at your POZ.

## In Ukraine

In Ukraine, people aged 40 and over can get a free cardiovascular risk check in one visit under the national Health Screening 40+ programme. The Public Health Center says a lipid profile measures total cholesterol, LDL, HDL and triglycerides, and results should be read with a doctor.
